There are four main phases of cardiac rehabilitation, each of which provide support and encouragement to patients with established heart disease. The primary aim of cardiac rehabilitation is for patients to achieve and maintain the best physical and psychological health possible.
The process of cardiac rehabilitation is offered to patients following a heart attack, heart by-pass operation and valve surgery, newly diagnosed angina, a coronary angioplasty and stent procedure, and of course for those patients who have a referral from a GP.
- Phase I cardiac rehabilitation is the in-patient phase for patients admitted to hospital after having a heart attack or recovering from a heart operation.
- Phase II cardiac rehabilitation is for when patients leave the hospital to return home under the care of their GP.
- Phase III cardiac rehabilitation patients are invited to a 10 week exercise and education programme overseen by a Cardiac Rehabilitation Nurse and a qualified Physiotherapist who will offer an individualised exercise programme. There are group discussions on various topics including lifestyle and stress management, dietary advice and blood pressure monitoring.
- Phase IV cardiac rehabilitation, as offered by the team at Glasgow West Heartbeats, is focussed on the long-term maintenance of a healthier and more active lifestyle.
